Memo on Performance Appraisal Results

Regarding your behavior throughout the post-probationary phase, I have given it much thought. As specified in your contract letter and in accordance with our organization's policies, the Human Resource team conducted an on-demand performance evaluation. This activity fell under our purview. Any new employee must successfully complete this test with an average score of 50%. The exercise's results were as follows:

As compared to other employees, an analysis on individual time-keeping assessment has ranked you the worst performing in terms of time management. As such, you only managed to score 39% out of the expected 50% pass-mark on this function.

Competency & Professionalism (CP)

Your job calls for proper professional licensing as well as competency as outlined in the organization policies provided to you when you were shortlisted for interview. We are pleased that your credentials, recommendations as well as zero record of non-accountability while on probation portray this aspect. However, despite your high professional achievements, your supervisor severally noted that you made mistakes beyond average error, based on your job description. Consequently, you garnered 50% in competency and professionalism.

Productivity and Commitment (PC)

Our entity is based on the foundation of strong employee commitment and client satisfaction as cited in the company’s objectives. Citing your habitual tendency of extending our humble lunch-break beyond the required time, I hereby point out that your regular absence from the work-station has affected the company significantly. Moreover, you have been noted to swap the firm’s productive hours with phone conversations with family and friends. As such, you attained 45% out of the possible 100% with respect to commitment and productivity.

Team-Work (T)

With great concern, I would like to point out that you have disassociated yourself from collaborative effort. Fellow colleagues who wish to remain undisclosed have independently reported in writing that there is lack of attention to your job. Subsequently, employees have begun to talk openly and resentfully about this matter. As our policies consider bi-annual grading of team-work, a period for which you have not stayed in the firm, we only decided to note down this effect and raise your attention to it.

Recommendation

The panel recommended that you consider attending time-management classes in addition to enhancing your competency and productivity and/or efficiency rate to be considered for a future position in the entity. However, in consistence with Fair Labor Standards Act and your contract letter, upon citing your average score, I herein regret to state that you have not attained the minimum requirements to proceed as our employee with immediate effect.
